Things to Remember

Vital INformation

The following information is needed for the death certificate and therefore must match the birth certificate.

  1. Decedent's full name (Including maiden name if female)
  2. Date of birth, Place of birth, Social Security number and Current address
  3. Father's full name and mother's full name (with maiden name)
  4. Marital status at time of death.If married - full name of spouse, date and place of marriage;If widowed - full name and year of passing
  5. Education level, Occupation and Place of employment
  6. Military discharge information and DD 214 papers
  7. Informant/Next of Kin: full name, address, contact phone number

Additional information needed

  1. Insurance policy/policies.
  2. Copy of marriage license, Cemetery deed, Power of Attorney documentation
  3. Religious preference, Name of church home, Minister name(s) and Musician name(s)
  4. Information for funeral programs (typed); suggested printing companies provided upon request
  5. Location of repast and Contact person
  6. Picture for funeral program and/or Newspaper obituary
  7. Photos for Memorial Tribute Video (16 photos)

Clothing, Jewelry and accessories

Your loved one will require full burial attire and a complete set of underclothing. Shoes/slippers are optional.

FEMALES:

  • Burial attire must be long-sleeved. Any lace or shear clothing must have a full heavy lining.
  • Underclothing - panties, bra, pantyhose, full slip or half slip with camisole
  • The neckline of the burial attire should not be low
  • Preferred nail polish and lip color (optional)
  • Preferred fragrance or scent

MALES:

  • Suit, tie and belt
  • Long sleeve shirt (long sleeve shirts with cuffs must have cufflinks)
  • Underclothing - briefs, undershirt/t-shirt and socks
  • Preferred fragrance or scent

ADDITIONAL NOTES:

  • If jewelry is to be used it will remain with the deceased. Costume jewelry is suggested. This also includes Rosary Beads. Please note that Mabrie Memorial Mortuary will not be held liable for the loss of any jewelry or personal items of the deceased.
  • Eyeglasses, wigs/hairpieces or other accessories should be brought along with clothing
  • Recent photo for hairstyling and cosmetology.

Notes and Special Instructions

Payment may be made in the following ways: Cash, Cashier's Check, Insurance Assignment, Pre-Need Funeral Plans, VISA, MasterCard, Discover and American Express. Personal Checks are not accepted.
